anchor – bolt has come out of concrete. How to fix it?

Have fixed a Basket Ball hoop ( not using the wooden board) directly to a concrete wall, using anchor bolts ( roughly 2-3 inches deep and < 1/2 inch dia).

However one of the anchor bolt has come out of place, after playing for few days. I guess either the hole was big or the anchor was not able to grip properly and the hole lining was giving away. not sure.

Now my requirement is to fix the loosened anchor. Need your advice.
(or better advice on fixing basketball hoops :-) )

Putting it in another place is not an option, for I need to drill 4 more new holes, which I dont want to do ( in current location, my other 3 bolts are in place and are secured good). I can continue with the as is existing structure, but guessing that the vibrations, would slowly damage the other 3 anchors as well, hence want to fix is asap.

Purchase a caulking tube that comes with a special mixing nozzle – it’s epoxy anchoring adhesive. You just need this epoxy and a regular caulking gun.
Remove the goal so you can get this epoxy back in the hole around your 1 anchor bolt that did not hold well. Make sure the hole is cleaned out good prior to squirting epoxy in the hole and around the bolt. This epoxy sets pretty quick but you will need to hold it in place so it does not run out of the hole. Just take a pc. of cardboard and make hole in center so it fits around the bolt. Squirt epoxy in then push carboard over bolt so its against wall, and hold it on for 5 minutes.
Pull off cardboard, wait 24 hrs. – Remount goal and install nuts washers on all bolts – tighten down snug – wait 1 more day – then really crank the bolts tight.
Do not get the epoxy on your hands or anything else you don’t want permanently globbed up with it.
